کاربردهای نوین کیتوزان در پانسمانهای زخم: یک راهکار مؤثر در درمان خونریزیهای تروماتیک

خونریزی ناشی از ضربه یکی از دلایل اصلی مرگومیر در حوادث، سوانح و جنگها بهشمار میرود و بهویژه در مواردی که خونریزی شدید و غیرقابل کنترل است، میتواند در مدتزمان کوتاهی منجر به شوک هموراژیک و مرگ شود؛ در این میان، استفاده از مواد هموستاتیک نوین مانند کیتوزان بهعنوان روشی مؤثر و سریع برای کنترل خونریزی مورد توجه قرار گرفته است. کیتوزان، که از کیتین موجود در اسکلت سختپوستان بهدست میآید، یک پلیساکارید زیستسازگار با بار مثبت است که از طریق برهمکنش الکترواستاتیک با سلولهای خونی (دارای بار منفی) موجب تجمع سلولی و تشکیل لخته پایدار میشود و حتی در شرایطی که فرآیند انعقاد طبیعی مختل شده باشد، میتواند با سرعت بالا خونریزی را مهار کند. این ماده نهتنها به دلیل عملکرد سریع و مؤثرش در بند آوردن خون مورد استقبال قرار گرفته، بلکه بهدلیل خواص ضدباکتریایی، ضدالتهابی و تسریع در روند ترمیم زخم، جایگاه ویژهای در طراحی پانسمانهای پیشرفته پیدا کرده است؛ بهطوریکه محصولات مختلفی همچون پودر Celox، پانسمان HemCon و گاز ChitoFlex بر پایه آن تولید شده و در میدانهای جنگ و شرایط اورژانسی به کار گرفته شدهاند. با این حال، چالشهایی مانند عملکرد محدود در زخمهای عمیق یا بیماران با اختلالات انعقادی باعث شده پژوهشگران نسل جدیدی از ترکیبات کیتوزانی را توسعه دهند که با افزودن نانوذرات، پلیمرهای هوشمند یا ترکیبات طبیعی مانند اسید تانیک، خواصی چون رهایش کنترلشده دارو، پرکردن مؤثر زخمهای نامنظم و قدرت بیشتر در مهار خونریزی و عفونت را ارائه دهند؛ در نتیجه، این پیشرفتها نویدبخش استفاده گستردهتر از کیتوزان در درمانهای اورژانسی و بهبود زخمهای پیچیده در آیندهای نزدیک هستند.
