.بررسی خطاهای پزشکی و عوامل مرتبط با آن در بیمارستان جامع زنان کوثر ارومیه از سال 1400-1403

زمینه و هدف: ایمنی بیمار یکی از مهمترین ابعاد کیفیت مراقبتهای بهداشتی-درمانی است و خطاهای پزشکی به عنوان چالشهای حیاتی نظامهای سلامت شناخته میشوند. این مطالعه با هدف بررسی خطاهای پزشکی و عوامل موثر بر آن در بیمارستان جامع زنان کوثر ارومیه انجام شد.
روش بررسی: این مطالعه توصیفی-مقطعی تحلیلی بر اساس دادههای ثبت شده در واحد ایمنی بیمارستان در بازه زمانی فروردین 1401 تا شهریور 1403 انجام گرفت. دادهها شامل نوع خطا، مسئول خطا، شدت پیامدها و وقایع مرتبط بوده و با استفاده از فرمهای جمعآوری اطلاعات و با نرمافزار SPSS نسخه 21 تحلیل شدند.
یافتهها: در مجموع 188 مورد خطا گزارش شد که بیشترین خطاها در بخشهای اتاق عمل (41.2%) و زایمان (36.2%) رخ داده بود. بیشترین خطاها در شیفت صبح برای اتاق عمل و شیفت شب برای بخش زایمان گزارش شدند و تفاوت معناداری بر اساس زمان شیفت مشاهده شد (P<0.05). نوع خطای جراحی با 39.4% بیشترین فراوانی را داشت. همچنین رزیدنتها مسئول 66.5% از خطاها بودند. شدت پیامدها عمدتاً عوارض موقت بود( 42%(.
نتیجهگیری: یافتهها نشان میدهد که مدیریت دقیق شیفتهای کاری، ارتقای آموزش و رفاه کارکنان و بهبود فرآیندهای سیستماتیک با استفاده از پروتکلها، در کاهش خطاهای پزشکی و ارتقای ایمنی بیمار نقش کلیدی دارد. انجام مطالعات گستردهتر در بیمارستانهای مختلف جهت تعمیم نتایج توصیه میشود.
